Please complete the recipient application and submit it electronically. Someone from Cleveland Conception Connection will contact you to give you a free user name and password. This will allow you unlimited access to the donor list. You will be able to view their photos and view their applications. You will be provided all non-identifying information and this will allow you to choose the donor of your choice.

Once you have selected a donor, please click on the link "Choose this donor" at the top of the page to fill out the donor selection form on the website. We will notify you as to whether the donor is still available and if she has not already been chosen.

After we have confirmed the donor is available you will download, sign and either fax or send the donor agreement form. In addition a fee of $800.00 will be due at this time, payable by check or credit card. The $800.00 covers a pyschoeducational evaluation of the recipient couple and a psychiatric evaluation and psychological test of the donor. If the donor is married, her husband will be required to be involved in the psychiatric evaluation and may be required to take the psychological test.

After the evaluations are completed you will sign the donor acceptance form B and the entire contract and fax it or send it in the mail. At this point you will send $6,000.00 for the agency fee and $5,000.00 for the donor fee to Cleveland Conception connection, Inc. Once we receive all payments and documents we will send the entire donor's information to your infertility specialist. We will have the donor schedule an appointment with your doctor where they will perform all necessary blood tests and medical exams and they will coordinate the donor's cycle with yours.
